What Are Case Management Services?

An important part of a comprehensive treatment program includes Case Management Services. Case Management begins with a “needs” assessment. Our patients work with a Case Manager to identify issues that could hamper their recovery.

The Case Manager and patient work collaboratively to develop a service plan that addresses any identified issues. The Service Plan is an individualized, goal-oriented plan to achieve specific measurable objectives.

Examples of services may include but are not limited to:

  1. Assistance with accessing support in the community.
  2. Linkage to formal community services.
  3. Symptom monitoring.
  4. Coordination and assistance with crisis management and stabilization.
  5. Advocacy and outreach.

Case Management Services are available to all clients during business hours Monday-Friday by appointment. These services may be provided in person or via telephone.
